Requirements for Submission of AAP

For a child born in the Philippines where the father is currently residing in Alberta or Saskatchewan, Canada, the AAP shall be notarized and registered at the Philippine Consulate General.

If the father of the child is in the Philippines but the birth occurred in Alberta or Saskatchewan, Canada, the AAP shall be notarized and registered at the Local Civil Registry Office of the place of execution of the document in the Philippines.

The Local Civil Registrar’s (LCR) Office or the Philippine Consulate General, whichever is applicable, shall also issue an accompanying Certificate of Registration in relation to the AAP.

Documentary Requirements for the Submission of the AAP:

  • Four (4) duly accomplished AAP by the father (write the first name, middle name, and last name); AAP form can be found here.
  • Four (4) photocopies of the birth certificate of the child issued by the Philippine Statistics Authority (PSA) or LCR.
  • Original and four (4) photocopies of the father’s passport and 1 government-issued ID with the father’s signature.
  • Prepaid return envelope (preferably with tracking number).

Fee of CA$36.25 for the AAP and CA$36.25 for the Certificate of Registration. Payment can be made in cash/debit/credit (with a 2.4% convenience fee applied for credit card only) if the application is submitted in person.

Submission by Mail:

  • Four (4) AAP executed by the father (first name, middle name, and last name must be written) must be notarized by a local Notary Public in Alberta or Saskatchewan and individually authenticated by the Deputy Provincial Secretary’s Office in Edmonton or Ministry of Justice in Regina, whichever is applicable.
  • Four (4) photocopies of the birth certificate issued by the Philippine Statistics Authority (PSA) or LCR.
  • Four (4) photocopies of the father’s passport and 1 government-issued ID with the father’s signature.
  • Prepaid return envelope (preferably with tracking number).

Fee of CA$36.25 for the AAP and CA$36.25 for the Certificate of Registration in Bank Draft or Postal Money Order, payable to the Philippine Consulate General (in exact words and no abbreviation).

Processing:

Please note that processing of the documents takes seven (7) business days, counted from the next business day following receipt of application, before the documents are returned by mail to the applicants, provided the requirements and procedures listed above are complied with.
